The quote form collects your contact details, address, date of birth, requested services, insurance details, contact preferences, typed signature, and Social Security number. You may also provide your driver’s license number and other information relevant to your request.

Kavuvu Insurance Broker LLC uses your information to review your insurance needs, prepare quote requests, and contact you. To obtain quotes, your broker may provide relevant information to insurance carriers or quoting platforms. Those services have their own privacy practices.

Storage and broker access

Submissions are stored in the website’s database. The broker inbox requires an authorized owner sign-in. Social Security and driver’s license numbers are encrypted when saved and masked in the inbox until the authorized broker reveals them. Other quote information is available to the authorized broker for review.

What to send

Use the designated fields for Social Security and driver’s license numbers. Do not include them in notes or email. Do not submit payment details or medical records through this form.
